CONGRATULATIONS! You have decided to send your child to the #1 Golf And #1 Tennis Camps in America!

Now what? Below you will find a list of forms and documents that must be sent to us by the date indicated. Most of these forms (except the Bunk Request Form) can also be found in the Parent Information Packet. But we have provided copies of just the individual forms for your convenience. Note: All personal information provided for camp will be held strictly confidential and not shared with any one else. Please safeguard your tuition with one of many Tuition Refund Insurance services found on line. 

We suggest that you consult your insurance agent to make sure that your child’s equipment and personal belongings are covered while in transit or at camp. The camp is not responsible for such loss or damage by fire, theft, laundry, etc.
