Clubs, appropriate for your child's size. Good while the supply lasts! Request
to the Maine Golf and Tennis Academy on the completed application form for the
appropriate size and color code of the set of U.S. Kids Golf Golf Clubs when
you enroll. Please click here to see US Kids Golf web site to learn about the
different sizes and clubs. Each set has anywhere from 6-10 clubs, depending on
level, plus golf bag. If you have any questions about the right size ask our

We request that
you use, David's Travel, to coordinate all the
travel plans, to insure the maximum convenience
for both your camper and the camp. Flights and
accommodations will be arranged at the lowest available
rates while still fitting into the most convenient
and appropriate times, dates and locations for
safe, smooth and convenient travel for your campers
arrival and departure at camp.

TERMS and CONDITIONS: Please
print out a copy for your records before you submit.
4 Week Session Tuition includes,
all greens fee, meals, room and board, medical first
aid attention, (does not include medicine, prescriptions,
doctor or hospital visits, medical treatment, medical
operations or procedures, x-rays or any medical treatment
that can not be appropriately supervised or administered
by our Medical Staff on duty, as determined by the
Director of the Maine Golf and Tennis Academy); Also
includes trips i.e. whitewater rafting, lobster feast,
mountain climbing, rock climbing, whale watching, sea
kayaking, and camp activities that have been scheduled.
A transportation fee of $85 one way, to and from Portland
Airport if arrival or departure is between 11:00 AM
and 3:00 PM on our scheduled arrival Sundays and our
departure Fridays, listed on the application form. To
and from (BOS) Boston Logan Airport is $175 per person
each way.
All campers must have adequate
medical insurance to cover all medical expenses incurred
while at camp.
All and final payments are due on
May 1st of the current camp year.
Current Camp Year is
defined as being the 12 months of time containing
the summer of the campers enrollment. It is between
September 1st through the following August 31st,
during which time a camper would be enrolled in one
or more of the camp sessions to be attended during
the summer ( June, July, August) during that same
period of time.
REFUNDS
Full refunds, for the amounts
paid, for the withdrawal or cancellation of a Maine
Golf and Tennis Academy (MGTA) camper, or of Family
Campers, if received in writing prior to April 30th
of the current camp year, will be allowed, less the
$1,000 administrative fee, which is non-refundable
and non-transferable. If withdrawal or cancellation
is received in writing before January 1st of the
current camp year there will be a refund for amounts
paid less a $350 administrative fee.
There are no refunds of any kind on or after April 30th of the current
camp year.
The current camp year is defined as the summer for which an
application form and enrollment fee has been paid, in part or in
whole, for a child to attend one or more of our sessions for that
same year, which are listed in our materials or online on our web
page, and as such, has been reserved for that child named on the
Application and Enrollment Form.
Please note there will be no refunds of any kind for early departure
or late arrivals. Refunds for withdrawal or change in arrangements
of any kind that have been submitted in writing before May 1st, of
the current camp year, will be given for amounts paid less a $1,000
administrative fee. If withdrawal or cancellation is received in
writing before January 1st of the current camp year, there will be
a refund for amounts paid less a $350 administrative fee.
There are no refunds of any kind on or after
April 30th of the current camp year. .

Withdrawals, additions, and cancellations of MGTA campers,
or their changes of their status, regardless of when, must be in
writing. No reductions, allowances or credits against fees will be
made for late arrival, early withdrawal, or dismissal of any camper.
Campers that convert from one length session to another, will pay
the fee quoted of the shorter time charged calculated at the time
of the change, and will forfeit all early-enrollment credits.Withdrawals
and cancellations of campers after April 30th of the current camp
year will be without refunds or credits
Maine Golf and Tennis Academy,
its owners and directors reserve the right to dis-miss
for cause any camper whose conduct or influence is
unlawful, in violation of camp policy, or is deemed
unsatisfactory to the camp community. In such event,
there will be no reduction or refund of camp fees.
MGTA, its owners and directors reserve the right
to use any individual or group photographs, video,
digital, and/or audio recordings, taken at Camp or
elsewhere, documenting campers in Camp activities,
for publicity and public relations purposes, including
publication on the MGTA website, both now and in
the future.
Canteen Enhancement Fee *
Canteen enhancement fees is used
by the camp management on behalf of the
camper for several things: pizza, outings,
day trips, food and snacks at the camp
store, sundry items such as stationery,
postage stamps, shampoo and batteries.
It is also used for special activities
outside normal camp programs such as:
bowling, ice skating, amusement parks,
miniature golf, day trip excursions,
movies, etc. The canteen enhancement
fee is a mandatory charge and is non-refundable.
Bedding and linen are
available to rent for an additional charge ($100
per month). This service includes blankets, sheets,
pillows. Sleeping bags are not included.
Club Rentals are $100
for two weeks and $125 for four weeks.
Transportation:
We are happy to provide transportation to and from
designated locations and airports. The fee and pick
up or drop off service for your child will
be calculated by time of day and location of the arrival
or departure of your child and then, only, upon receipt
of your request in writing on our Transportation Form,
by April 15th of the current year.
Please take advantage of
our Pick Up and Drop Off from Portland Maine's
Airport for a small addtional fee:
We will provide, pick up and drop off plus transportation to and
from Portland International Airport (PWM), Portland, Maine, for
campers scheduled to arrive or depart between 11:00 AM
and 3:00 PM on the scheduled arrival Sundays and departure Fridays
for $85 each way.
Other times, dates, places
or special transportation needs: Any other
transportation that we provide, must be arranged
in writing by the parents on our Transportation
Form and submitted no later than April 15th of
the Current Camp Year. If the camper will require
transportation that will be at times, dates or
places other than the above schedule for (PWM)
there will be an extra charge based on time, date,
and place.
All camper's fees and amounts due must be paid in full before they
will be permitted to attend camp.
By filling out this application
form and sending it to the Maine Golf Academy, I
agree, that by submitting this application for enrollment
form, it is my electronic signature and my agreement
to the all the Terms and Agreements, and there will
be no charge back for any reason or any amount, and
I consent to allow all scheduled payments by credit
card, unless paid otherwise and in the event that
payment has not been received by the camp upon or
before the scheduled payment dates, to be charged
on my credit card at the time of and on the payment
Schedule listed below:
$1,000 per each two weeks upon
deposit.
$1,000 per each two weeks on
January 15th of the current year
Final Payments due on April
30th of the current year
This agreement, and any and
all disputes or claims, including but not limited
to: credit card charges or payments, wire transfers
payments and check payments, against the Directors,
the employees, agents or the Kennebec Boys Camp,
Inc. dba Maine Golf Academy shall be interpreted,
governed and constrained by laws and courts of The
State of Maine, Kennebec County, and all legal actions
shall be under the jurisdiction of The State of Maine
in Kennebec County.
